Yes, at my previous workplace a girl working with me was injured. It was our responsibility to take horses out of their stalls, ride them, brush them, and clean their stalls and she was not being careful and accidently spooked a horse causing it to step on her foot and break some of her toes. This could have been easily prevented by simply taking better safety precautions.
Injury in the workplace can almost always be prevented by simple taking more, or better, safety precautions and simple paying attention to what it is you are doing. Employees must be able to think clearly and not be rushed when in potentially dangerous scenarios.
